Health Care Connect

Local Family Health Teams Accepting New Patients

The Kingston and Maple Family Health Teams are currently accepting new patients who currently do not have a family physician.  A Family Health Team is an approach to primary health care that brings together different health care providers to co-ordinate the highest possible quality of care.  Designed to give doctors support from other complementary professionals, most Family Health Teams consist of doctors, nurses, nurse practitioners and other health care professionals who work collaboratively, each utilizing their experience and skills.  For more information on health teams, please visit the Ministry of Health and Long term Care website.

For information on local clinics, locations and practicing physicians, please visit the Health Teams websites:

College of Physicians and Surgeons of Ontario

The College of Physicians and Surgeons of Ontario has an online family doctor database which allows you to search for information about family physicians, including their practice addresses, telephone numbers, qualifications and whether they are accepting new patients.  Please visit their website at: http://www.cpso.on.ca/docsearch/
